Puppy Plan
The Wellness Puppy Plan is for puppies one year old or younger.
This plan includes the first year of vaccines (Da2PP/Da2PP4L; Bordetella; Rabies), intestinal parasite screenings and the accompanying treatment, basic blood work with a heartworm test, HomeAgain Microchip (first year registration paid), 4 complimentary Heartgard and Nexgard with your first four office visits and unlimited complimentary nail trims
Canine Adult Plan
The Wellness Canine Adult Plan is for dogs one to six years of age.
This plan includes an annual exam, Da2PPv, Leptospirosis, Rabies and Bordetella vaccines, intestinal parasite screening and the accompanying treatment, Wellness blood work including Heartworm and tick-borne disease screening.
Canine Senior Plan
The Wellness Canine Senior Plan is for dogs six years of age or older.
This plan includes an annual exam, Da2PPv, Leptospirosis, Rabies and Bordetella vaccines, intestinal parasite screenings and the accompanying treatment, wellness blood work including thyroid test, a urinalysis, heartworm and tick borne disease screening.
Kitten Plan
The Wellness Kitten Plan is for kittens one year old or younger.
This plan includes the first year of vaccines Feline Leukemia, FVRCP and Rabies, intestinal parasite screening and the accompanying treatment, Wellness blood work and feline leukemia/FIV testing, 4 complimentary doses of revolution with your first 4 office visits, HomeAgain Microchip (first year registration included) and unlimited complimentary nail trims.
Feline Adult Plan
The Wellness Feline Adult Plan is for cats one to six years of age.
This plan includes an annual exam, FVRCP, feline leukemia vaccine and rabies, intestinal parasite screening and the accompanying treatment, Wellness blood work and feline leukemia/FIV testing.
Feline Senior Plan
The Feline Senior Plan is for cats six years of age or older.
This plan includes an annual exam, FVRCP, feline leukemia vaccine, rabies, intestinal parasite screening and the accompanying treatment, Wellness blood work including a thyroid test, urinalysis and feline leukemia/FIV testing.
Things You Need to Know:
This is not insurance- it is a wellness plan that covers all things wellness in a 12-month
payment plan.
Initial membership of $59.95 ($20 off after the first pet)
· After the first 1st payment, your plan’s monthly cost will be automatically withdrawn on
the 20th of every month.
· You may add additional service items specifically for your pet for an additional price.
· Each plan is for a period of 12 months and will automatically renew unless otherwise
stated.
· You may cancel your Petly Plan at any time; however, you are responsible for the cost of
any services your pet received. We will require the cancellation request in written form.
· Any payments made prior to cancellation are non-refundable.
